It's not on App Store. It's an ad hoc distribution. Search for 'i-KeyHoleTV_OS3' on Google and you should be able to find it.

I'm not really sure what this website is but it's known for streaming japanese tv and maybe other stuff, sometimes all channels work and most of the time they're down with only 2 or 3 streams working.

I've been using this link for a while, it's not HQ but it really helps when all of gundam's streams are down

To make it simple, yes it is a VOD application using windows media player.
If you need more informations, have a look at their website, I also think people were discussing about it around pages 40/43 of this very topic or in another one there.

If you want to watch Inazuma Eleven from TV Tokyo just use the kansai streams, there is a channel called TVO, it's the same schedule so you can enjoy it.
